What Michel wants to discuss

“I want my hands to work in the late morning, so I can get dressed and cook without waiting.”

Michel, in his words

  1. My hand is stiff before the 11:00 tablet. Should we look at the timing?
  2. My voice gets quieter in the evenings. Is that worth looking at?
  3. Can you see my blood-pressure and diabetes medicines alongside these?

Sophie, his wife, asksCan we ask whether the late-morning stiffness fits the new timing?

What it gives the consultation

The patient’s priority first

Their functional goal and their questions open the brief.

Accounts kept separate

Each entry shows whether it was observed or reported, and by whom: the patient or a caregiver.

Timing you can use

When a symptom was noted, against the dose schedule, before and after a medicine change.

The whole medication picture

Every medicine, every prescriber and coexisting conditions, on one record.

Before and after a change

Levodopa went from three to four doses a day on 3 September. The brief places the noted stiffness times on either side of that change: 11:45 to 12:10 before it, 10:05 to 10:40 after. It shows the pattern without interpreting it. Interpretation stays with the clinician.

How to read the brief

Provenance

Each entry records its source (patient, caregiver, device or clinic) and whether it was observed or reported.

Timing

Event time and recording time are stored separately. Approximate times are labelled as approximate.

Adherence

Doses show as marked or unmarked against the plan. An unmarked dose is unconfirmed, not missed. Counts show what was recorded, not everything that happened.

Medicines

As the patient entered them, with prescribers. No clinician has reconciled the list.

Coding

Observations can map to SNOMED CT, with a record of how each mapping was made. A code doesn’t turn a patient’s report into a clinical finding.

Limits

Every brief states what the record cannot show. Sennit assigns no cause to any entry and makes no diagnosis or treatment suggestion.

Nothing new to manage: no clinician login, no dashboard and no alerts. The patient brings the brief, and nothing is sent to the clinic.
